Sporadic Hypercalcitoninemia: Thyroid Medullary carcinoma, Autoimmune Thyroiditis or GLP-1 Agonists’ Effect?

Introduction: Glucagon-like peptide 1 (GLP-1) receptor agonists’ stimulated C-cell hyperplasia and tumors in rodents, but not in humans. Calcitonin also increased in autoimmune thyroid disease.

Aim(s): To present difficulties in differential diagnosis of sporadic hypercalcitoninemia in a type 2 diabetes mellitus patient with autoimmune thyroiditis.

Materials and methods: M.S., woman, 66 years old, resident in a former iodine-deficient area, with type 2 diabetes mellitus, subclinical hypothyroidism (TSH=10 mIU/L) due to autoimmune thyroiditis (antiTPO Abs=2846 IU/mL), received GLP-1 agonist treatment for 18 months; there was no familial history of medullary thyroid carcinoma; calcitonin was measured by immunoassay.
