#1870 Detection of Metastatic Insulinoma with 68Ga-NODAGA-Exendin-4 PET/CT in a Young Child – A Case Report
Introduction: Malignant insulinomas are extremely rare in children. Localizing metastases of insulinoma is challenging. Imaging modalities like CT and MRI show limited results. Somatostatin-receptor (SSTR) imaging (SRS) performs better. However, a subgroup of malignant insulinoma lack SSRT-2 and can, therefore, not be localized using SRS. We propose that in these cases GLP-1R imaging, using the stable GLP-1 analog exendin, could play an important role.

#483 Efficacy of Everolimus in Patients with Metastatic Insulinomas and Refractory Hypoglycemia: For the French Group of Endocrine Tumors (GTE-Renaten)
Introduction: Refractory hypoglycemia in patients with metastatic insulinomas (RHMI) is an important cause of morbidity and mortality and Everolimus could be a new therapeutic option.

#502 Bifunctional ShRNAimPDX-1 Therapy Prevents Hypoglycemic Death From Insulinoma in Mice
Introduction: PDX-1 is a novel molecular target for insulinoma. Bi-functional shRNAi platform has been developed that results in a more effective targeted gene knockdown.

#2963 Intrinsic Gene Programs in Metastasis-like Primary Subtype Is Associated with Early Metastatic Evolution in Pancreatic Neuroendocrine Tumours
Introduction: Three molecular subtypes of pancreatic neuroendocrine tumours (PanNETs) have been described: insulinoma-like, intermediate and metastasis-like primary (MLP). The Latter is a non-functional tumours with highly metastatic potential. A mouse model of PanNET showed that MLP subtype has two different pathways of progression including stem/progenitor and epithelial-to-mesenchymal transition. However, the evolution of metastasis in patients requires further studies.

#2003 Comparison of Glucagon-Like-Peptide-1 (GLP-1) Receptor PET/CT, SPECT/CT and 3T MRI for the Localisation of Insulinomas: Evaluation of Accuracy in a Prospective Crossover Imaging Study
Introduction: Benign insulinomas are the most prevalent cause of endogenous hyperinsulinaemic hypoglycaemia (EHH) in adults and due to their small size difficult to localise.

#2223 Diagnostic Dilemma in a Patient with Recurrent Hypoglycaemia: An Unusual Case
Introduction: Recurrent hypoglycemia is a part of Whipple’s triad and usually points towards insulinoma as a cause. Presence of metastasis is uncommon in Insulinoma, and other causes should be ruled out.

#2249 Pasireotide as a Possible Tool for the Control of Refractory Hyperinsulinemic Hypoglycemia from Malignant Insulinoma
Introduction: The control of hypoglycemic hyperinsulinemic syndrome from malignant insulinoma is challenging because it is often refractory to diazoxide and somatostatin analogues (SA). Everolimus can have side effects that can limit its use. Pasireotide, a multi-receptor-targeted SA with a high binding affinity to SSTR1 and SSTR5, exhibits a strong inhibitory effect of insulin secretion.
