Clinical Value of Somatostatin Receptor Scintigraphy in the Management of Medullary Thyroid Carcinoma Abstract #1299

Introduction: Medullary Thyroid Carcinoma (MTC) is a rare form of thyroid cancer. It represents 5 to 10 % of these cancers. MTC grows from specialized thyroid cells called para-follicular cells, or C-cells that secrete a hormone called calcitonin. Its prognosis is usually good. Recurrences can occur, requiring lifelong surveillance.
Aim(s): The aim of this study is to evaluate the clinical value of Somatostatin Receptor Scintigraphy (SRS) in the management of patients with MTC.
Materials and methods: Four patients (2 man, 2 women) aged 28-67 years underwent a total of five SRS. All these patients had a total thyroidectomy for MTC. SRS was requested during an assessment of post-operative extension in two cases, research of recurrence in two cases and therapeutic evaluation in the last case.

#80 Metastatic growth hormone secreting pituitary carcinoma treated with peptide receptor radionuclide therapy
Introduction: Pituitary carcinoma (PC) is an extremely rare condition defined by the presence of adenohypophyseal neoplastic tissue outside the pituitary. Clinical experience regarding diagnosis, management and prognosis of PC is very limited. Growth hormone (GH) secreting PC is even rarer and represents a particular challenge to clinical practice. Therapeutic modalities utilized to treat PC include surgery, radiation, hormonal therapy, and cytotoxic drugs. Peptide Receptor Radionuclide Therapy (PRRT) is an emerging therapeutic modality that involves the targeted delivery of an ablative dose of radiolabelled somatostatin analog. PRRT has been applied to various neuroendocrine tumors and results in prolonged survival and enhanced quality of life. As yet, this therapy has not been applied to malignant pituitary tumors.

#108 Everolimus (RAD001) induces tumor response in patients with medullary thyroid cancer: an in vivo and in vitro study
Introduction: Medullary thyroid cancer (MTC) is a neuroendocrine tumor (NET) of the thyroid C cells which is known to express somatostatin receptors. Octreotide, a somatostatin analogue exerting both antisecretory and antitumor activity in different types of NETs, has shown low efficacy in MTC. Everolimus (RAD001), an inhibitor of mTOR, has shown antitumor effects in patients with NETs and synergistic effects when combined with octreotide. Until recently, RAD001 has never been evaluated in MTC.

#156 Toxicity, response and survival analyses of Peptide Receptor Radionuclide Therapy (PRRT) in treatment refractory metastatic thyroid cancer using Yttrium-90 and Lutetium-177 Labeled somatostatin analogs
Introduction: Medullary thyroid carcinoma (MTC) represents about 3-16% of thyroid cancer. After first line treatment, therapeutic options for locally recurrent or metastasizing tumors include repeated surgery, whenever possible. Doxorubicin containing chemotherapy regimens in iodine-nonavid differentiated thyroid cancer have poor efficacy.

#160 The role of T-type calcium channels in medullary thyroid carcinoma (MTC): T-type calcium channel blockers inhibit hormone secretion and induce apoptotic cell death in a human MTC cell line
Introduction: Medullary thyroid carcinoma (MTC) accounts for approximately 5-10% of thyroid cancers. In the case of tumors limited to the thyroid gland, the prognosis is generally favorable, whereas 5-year survival averages only 40% in patients with metastatic disease. In these patients, conventional chemotherapy only occasionally leads to a complete response and partial responses are observed in less than 30% of the cases. Therefore, intense efforts are currently directed toward the identification of new druggable targets for the treatment of this devastating disease.
