Detection of cardiac metastases in patients with neuroendocrine tumors: comparison of Ga-68 DOTA-SMS-R PET/CT, cardiac MRI, CT scan and 2D echocardiography Abstract #159

Introduction: Neuroendocrine tumor metastases to the heart have been sparsely reported in literature. There is as of yet no detailed study on the best imaging method for the detection of this rare site of metastases.
Aim(s): To investigate the diagnostic accuracy of molecular, morphological and functional imaging modalities for the detection of heart metastases (HM) in neuroendocrine tumor (NET) patients.
Materials and methods: Overall, 713 progressive NET patients were examined with Ga-68 DOTA-Somatostatin-Receptor (SMS-R) PET/CT (using mainly DOTA-NOC and DOTA-TATE) and 2D echocardiography (echo). Twenty-eight patients had HM (incidence 4%). In 12 patients, cardiac MRI was also performed; FDG PET/CT was available in 24 patients. All imaging studies were reviewed separately by two independent experts. Cases were classified based on confirmation of HM in functional and morphological studies. The diagnostic accuracy of SMS-R-PET/CT, cardiac MRI, echo and thoracic CT scan studies was compared.

Introduction: Neuroendocrine tumors (NET) constitute a heterogenous group of neoplasms. The development of Gallium-68-labeled somatostatin analogues, such as DOTA-NOC or DOTA-TOC, PET/CT have dramatically improved the diagnosis of neuroendocrine tumors. Surgery remains the treatment of choice for localized disease as well as metastatic disease. The aims of surgery are: improvement of symptoms, reduction of tumor mass / burden and to give better quality of life to patients. Recurrent laparotomies often lead to multiple adhesions and altered anatomy. So it is increasingly difficult for imaging physicians and surgeons to separate non-malignant from malignant tissue.

#16 Endoglin as indicator of metastatic neuroendocrine tumors of the pancreas
Introduction: Neuroendocrine tumors of the pancreas are rare, highly vascularized tumors. Endoglin, a Transforming Growth Factor-β co-receptor, is a marker for angiogenic endothelial cells. Angiogenesis is required for tumor progression and the development of metastases. Recently, endoglin expression was found to be a prognostic marker in pancreatic carcinomas. However, the role of endoglin in neuroendocrine pancreatic tumors has so far not been studied.

#47 Plasma CCN2/connective tissue growth factor is associated with right ventricular dysfunction in Patients with Neuroendocrine Tumors
Introduction: Carcinoid heart disease (CHD) is a known complication of neuroendocrine tumors (NETs), particularly of those arising from the small intestine, appendix and proximal colon (previously known as mid-gut carcinoids). CHD is characterized by right heart fibrotic lesions and has traditionally been defined by the degree of valvular involvement, most commonly in the form of tricuspid regurgitation. Right ventricular (RV) dysfunction due to mural involvement may also be a manifestation. Connective tissue growth factor (CCN2) is upregulated in many fibrotic disorders. Increased tumor expression of CCN2 has been shown in patients with small intestinal NETs associated with peritoneal fibrosis. At present, its role in carcinoid heart disease is unknown.

#59 Incidence and clinical significance of adrenal masses in patients with gastrenteropancreatic neuroendocrine tumors
Introduction: The widespread application of modern imaging modalities, mainly computerized tomography (CT) and magnetic resonance imaging (MRI), has revealed a 2-3% incidence of inadvertently discovered adrenal masses, the majority of which are non-functioning benign adrenal adenomas. In the presence of a known malignancy, such lesions have a more than 30% incidence of being metastases. As patients with gastrenteropancreatic neuroendocrine tumors (GEP-NETS) have mainly well-differentiated and slow-growing tumors and are subject to routine abdominal imaging for disease staging or during follow-up, it is important to study the incidence and significance of such lesions based on the recently introduced TNM classification system.
