Ectopic Neuroendocrine Carcinoma: An Occult Source of ACTH

Introduction: About 250 cases of ectopic ACTH-producing neuroendocrine carcinoma (ACTH-NEC) have been reported; however metastatic ACTH-NEC of unknown primary is very rare. Usually are characterized by having a large cell, small cell, poorly differentiated or anaplastic cell, and Ki-67 index between 40-90%.

Aim(s): Exhibit the challenge that represents the workup and treatment of occult ectopic ACTH-NEC.

Materials and methods: We present a case of a 67-year-old male with hypokalemic alkalosis. He complains asthenia, adynamia, hyporexia, fatigue, weakness, weight loss of 5 kg, polydipsia, diabetes mellitus of 3 weeks of evolution, absence of a Cushing phenotype. He was a smoker.
