Germline Variants in Cancer Predisposing Genes in Young Adults with Neuroendocrine Tumors Abstract #2816

Introduction: Advances in genomics have enabled the recognition of new cancer predisposing genes (CPG). There are few studies of CPG in neuroendocrine tumors (NET).
Aim(s): To define the frequency of pathogenic (PV) and probably pathogenic (PPV) germline variants in known CPGs in young adults with NET.
Materials and methods: We prospectively screened consecutive patients (pts) with lung or digestive NET diagnosed under 40 years without known history of cancer hereditary syndromes for germline variants in 113 CPG of high to moderate penetrance. Sequencing was performed through next generation sequencing (NGS -NextSeq 500, Illumina), using a custom panel with IDT LockdownProbes. Alignment and variant calling were performed with the Isaac Enrichment v2.1 tool. Identified variants were filtered by the VarSeq software (Golden Helix) and classified using the American College of Medical Genetics criteria. When PV and PPV were detected in genes without known association with NET, we evaluated related tumors for gene expression alterations and loss of heterozygosis of the wild allele. Clinical data were collected.

#2712 HORMONET: Study of Tamoxifen in Well Differentiated Neuroendocrine Tumors (NET) and Hormone Receptor Positive Expression
Introduction: We have shown immunohistochemistry (IHC) expression of estrogen (ER) and progesterone receptors (PR) in 20.8% and 18.8% of 96 NET patients (pts). An old clinical trial (Mortel C, 1984) suggested antitumor effects of estrogen/progesterone-directed therapies in NET. Yet the effects of an antiestrogen agent in NET pts whose tumor express ER and/or PR are unknown.

#2817 METNET: A Phase II Trial of Metformin in Patients with Well Differentiated Neuroendocrine Tumors
Introduction: Preclinical studies have suggested that metformin has antitumor effects, likely due to blockage of mTOR pathway through AMPK and decreased insulin levels. A retrospective study showed that metformin combined with everolimus to treat related hyperglycemia offered longer progression-free survival (PFS) in patients (pts) with pancreatic neuroendocrine tumors (NET).

#2862 Concurrent Metastatic Pulmonary Neuroendocrine Tumor and Pituitary Macroadenoma
Introduction: The coexistence of a sellar mass in a patient with a neuroendocrine tumor, while rare, can paint the clinical picture of either a genetic syndrome, most notably multiple endocrine neoplasia syndrome type 1(MEN1) or pituitary metastasis recently more frequent probably due to the longer survival of cancer patients.
