Iodinated Exendin-4 Shows Improved Pharmacokinetic in Rat Insulinoma Model Abstract #1321

Introduction: GLP-1 receptor is an important marker for imaging benign insulinomas.
Aim(s): The limitations of existing PET tracers are high kidney uptake and low specific activity. We attempted to improve these by using radioiodinated (Ex-4).
Materials and methods: Kd and Bmax of [Nle14,125I-Tyr40-NH2]Ex-4 (125I-Ex-4) were determined in in vitro binding assay using Ins-1E rat insulinoma cells. Biodistribution of 125I-Ex-4 was studies in nude mice bearing rat Ins-1E xenografts.

Introduction: Biochemical diagnosis of endogenous hyperinsulinemic hypoglycemia is straightforward. Surgical removal of an insulinoma is hampered by difficulties to localize it using conventional radiological procedures. In vitro data suggest that human insulinoma cells exhibit a high density of GLP-1R. 111In-exendin-4 is an 111In labeled GLP-1R agonist that binds with high affinity to GLP-1R and may be helpful in localizing insulinomas.

#1870 Detection of Metastatic Insulinoma with 68Ga-NODAGA-Exendin-4 PET/CT in a Young Child – A Case Report
Introduction: Malignant insulinomas are extremely rare in children. Localizing metastases of insulinoma is challenging. Imaging modalities like CT and MRI show limited results. Somatostatin-receptor (SSTR) imaging (SRS) performs better. However, a subgroup of malignant insulinoma lack SSRT-2 and can, therefore, not be localized using SRS. We propose that in these cases GLP-1R imaging, using the stable GLP-1 analog exendin, could play an important role.
