Pancreastatin: A Predictive Neuroendocrine Tumor Marker Abstract #591

Introduction: Pancreastatin is a derived peptide of CgA. Pancreastatin has the potential to be a diagnostic and predictive tumor marker in detecting NETs.
Aim(s): To compare the predictive value of pancreastatin to CgA for identifying patients with NETs.
Materials and methods: Radioimmunoassay tests of pancreastatin and CgA were performed on 103 patient specimens collected from Mount Sinai Medical Center between 1/2010 and 7/2012. Patient demographics, diagnostic tests, surgical procedures, pathologic findings, adjuvant treatments, and survival were retrospectively reviewed. Statistical analysis utilized SPSS v20 software.

#386 A Prospective Evaluation of the Effects of Chronic Proton Pump Inhibitor Use on Plasma Biomarker Levels in Humans
Introduction: Proton pump inhibitors (PPIs) are used primarily to treat gastroesophageal reflux disease. PPI-induced achlorhydria increases circulating gastrin and chromogranin A (CGA). CGA is a widely used biomarker for the diagnosis and follow-up for gut-based neuroendocrine tumors (NETs). PPI-induced increases in CGA or gastrin may falsely suggest the presence of a NET when none exists. Pancreastatin, a fragment of CGA, is also commonly used to diagnose and follow NETs.

#152 Validation of a new assay that uses 2 monoclonal antibodies to measure intact and fragmented plasma CgA levels
Introduction: Plasma chromogranin A (CgA) levels are elevated in a variety of neuroendocrine tumors (NETs) and are used to aid in diagnosis, prognosis, and monitoring of treatment effects in NETs patients. We evaluated a newer sandwich ELISA assay (Cisbio US, Bedford, MA, USA) that utilizes two monoclonal antibodies that bind to the mid-region of the CgA protein and allow measurement of intact and fragmented circulating CgA. The results from the assay validation indicative of its overall robustness in terms of sensitivity, specificity, reproducibility, and stability are presented here.

#1543 Is True Non-Secretion of Chromogranin A an Unfavorable Prognostic Factor in Patients with ENETs TNM Stage IV Gastroenteropancreatic Neuroendocrine Tumors?
Introduction: Chromogranin A (CgA) is the best available serum marker for the work-up of gastroenteropancreatic NETs (GEP-NETs) and correlates with tumor volume & biological activity. During diagnosis & follow-up we found patients with elevated CgA levels and patients without elevated CgA levels (=‘true non-secretors’). We postulated that lack of secretion is a sign of dedifferentiation with a poorer prognosis.
