Systemic Markers of Inflammation in Neuroendocrine Tumors (NETs) and Outcomes with Everolimus: A Pooled Analysis from the Randomized, Phase 3 RADIANT-3 and RADIANT-4 Trials Abstract #2115

Introduction: NETs are extremely heterogeneous in nature. Despite recent advances, few biomarkers are available to consistently predict prognosis and optimize treatment selection.
Aim(s): The present pooled analysis assessed whether systemic inflammation is associated with poor outcomes among patients (pts) with NET by assessing neutrophil-to-lymphocyte ratio (NLR) and lymphocyte-to-monocyte ratio (LMR) at baseline.
Materials and methods: Pts with advanced, low-, or intermediate-grade pancreatic (p), gastrointestinal (GI), or lung NETs received either everolimus (EVE) 10 mg/day oral or placebo in RADIANT-3 (p; N=207, N=203) and RADIANT-4 (GI or lung; N=205, N=97) trials. Pts receiving EVE were grouped into NLR high (≥2.58) or low (<2.58) and LMR high (≥3.76) or low (<3.76) groups, based on the median values at baseline. Progression-free survival (PFS; central) was estimated by Kaplan-Meier method and hazard ratio (HR) using adjusted unstratified Cox regression model.

Introduction: The predictive value of tumor grade in the prognosis of carcinoid is insufficient and somehow misleading. The grade and differentiation in carcinoid tumors must be seen in conjunction with the Ki-67 index. The Ki-67 protein is a cellular marker for proliferation, present during all active phases of the cell cycle (G1, S, G2 and mitosis) and absent from resting cells (G0). It is therefore an excellent marker to determine the growth fraction of a given cell population. The Ki-67 labelling index is often correlated with the clinical course of carcinoid.

#34 Plasma Chromogranin A(CgA) and Urinary 5 Hydroxy-indoleacetic acid(5-HIAA) use in monitoring somatostatin analogue(SSA) therapy at Newcastle NHS Foundation Trust
Introduction: Two main biochemical markers, plasma chromogranin A (CgA) and urinary 5 hydroxy-indoleacetic acid (5-HIAA) are used to aid staging and monitoring of patients with neuroendocrine (NET) tumors. Somatostatin analogues (SSA) such as octreotide and lanreotide have been used to alleviate symptoms and disease progression in NET patients. This study evaluates the use of CgA and 5HIAA in monitoring NET patients on SSA therapy at Newcastle NHS Foundation Trust.

#41 Immunohisochemical evaluation of EMT regulators, E- and N-cadherin in neuroendocrine Tumors of the Gastro-Entero-Pancreativ system
Introduction: Local tumor invasion represents the first step of the metastatic cascade of carcinomas, and requires changes in cell adhesion and migration properties of tumor cells. This biologic process is known as epithelial-mesenchymal transition (EMT). One key biochemical change associated with EMT is the loss of E-cadherin expression promoted by specific transcriptional repressors such as Snail, Slug, and Twist. Overexpression of EMT inducers increases other factors, such as FoxC2, although its role in EMT is poorly understood. Neuroendocrine tumors (NETs) of the gastroenteropancreatic (GEP) system, originated from the diffuse endocrine system, represent a heterogeneous group of tumors. Their prevalence has increased substantially over the past three decades, without substantial improvements in their clinical management, and their variable clinical course cannot be predicted by common clinicopathological parameters. Thus, new prognostic markers are urgently needed.

#90 The prognostic value of FDG-PET scan in neuroendocrine tumors: a retrospective analysis of 46 patients treated in one center
Introduction: Neuroendocrine tumors (NETs) are rare and generally indolent. For diagnostic purposes, the sensitivity of FDG-PET scan is known to be low in this setting, though when positive, its prognostic value is not well-defined in NET.

#96 Expression of components of the mTOR pathway in gastroenteropancreatic neuroendocrine tumors
Introduction: Recently, the mammalian target of rapamycin (mTOR) inhibitors have entered late phase clinical trials in a broad variety of solid human malignancies, including neuroendocrine tumors (NETs). Since these drugs will certainly be used as routine therapeutics in the near future, it is surprising that information on the exact expression patterns of mTOR and its downstream target 4EBP1 in NETs is still lacking.
