#4666 Merkel cell neuroendocrine carcinoma of the skin: High response rates to short course palliative radiotherapy – Typical clinical scenarios

Introduction: Merkel cell carcinomas (MCC) are a rare, aggressive skin neuroendocrine carcinomas, with peak incidence in the elderly. MCC often presents as a firm, red/purple painless nodule with a short history of increasing size. Surgery is considered the 1st line treatment but elderly patients, in particular, have comorbidities which may preclude radical surgery requiring reconstruction. In advanced MCC, immunotherapy (IO) is 1st line systemic treatment. Chemotherapy is used 2nd line or if contraindications for IO but can be challenging in an elderly population. There may be an emerging role for PRRT due to the presence of somatostatin receptors on MCC. Radiotherapy (RT) can be used for non-surgical candidates, or those with unresectable or metastatic disease. Adjuvant post-operative irradiation (PORT) may provide additional benefit in risk reduction and improves local control.

#3273 LIBRETTO-531: Selpercatinib vs standard of care in patients (pts) with multikinase inhibitor-naïve advanced or metastatic RET-mutant (mut) medullary thyroid cancer (MTC)

Introduction: Multikinase inhibitors (MKI) are approved for MTC; however, their efficacy in RETmut MTC is limited due to incomplete RET inhibition and significant toxicity. Selpercatinib (LOXO-292), a highly selective and potent FDA approved inhibitor of RET alterations including M918T, MKI resistance-associated V804M, and others, showed robust, durable clinical activity with a tolerable profile in pts with RETmut MTC in the Phase 1/2 LIBRETTO-001 study.

#1248 Case Report of Recurrent VIPoma

Introduction: 51yr presented with 7 month diarrhoea&2 stone weight loss. All tests negative, only abnormality hypokalaemia. FGH profile: VIP raised>400, peptide histidine methionine>1000, GAWK of 250. Diagnosis of VIPoma. CT: 5cm mass tail & body of pancreas, no hepatic or adrenal lesions. Octreotide treatment started. Bowel frequency improved, hypokalaemia corrected. Octreotide scan showed uptake only in pancreas. Patient readmitted 5 months later hypokalaemic myopathy, K 1.7, surgical distal pancreatectomy & splenectomy completed. Histology: NET within pancreas, moderate cellular differentiation, variable mitotic rate.
