Introduction: Although rectal neuroendocrine tumors (NETs) with L-cell phenotype and small tumor size generally had better clinical behavior, the new 2019 WHO classification classified all rectal NETs are malignant without considering L-cell phenotype and small tumor size. Therefore, identifying biomarkers of rectal NETs is important to stratify the clinical behavior of rectal NETs.

Introduction: Pancreatic neuroendocrine neoplasms(NENs) are classified into neuroendocrine tumors (NET) G1, G2, G3, and neuroendocrine carcinoma (NEC), which are different pathogenesis. The two kinds of tumors that occurs in the same lesion in one patient is very rare. We found high grade neuroendocrine neoplasms(NENG3) with a diameter of 0.1 cm in a patient with a pancreatic NETG2 (1.7 cm in diameter). The cells were significant atypia with Ki67 index of 60%, focal necrosis, while metastatic nodules in the liver (12, 0.1-1.1 cm in diameter) were all high grade NENG3.

Introduction: Ki-67 is a nuclear protein that is strictly associated with cellular proliferation,acting as a cellular marker for proliferation in clinical practice to determine the degree to which the tumor cells are duplicating. Ki-67 index strongly ties to the prognosis and provides guiding significance to clinical decisions.
