Introduction: For advanced G1-G2 GEP-NET patients, peptide receptor radionuclide therapy (PRRT) with 177Lu-DOTATATE can be recommended. However, there are several SSTR-positive tumours for which PRRT has previously shown efficacy that are not currently covered by the indications for 177Lu-DOTATATE in Italy and, therefore, cannot benefit from it. Less used and studied is the 177Lu-DOTATOC. This is a radiopeptide with excellent safety and efficacy profile in PRRT and certainly not lower than 177Lu-DOTATATE.

Introduction: Well-differentiated gastroenteropancreatic neuroendocrine tumors (GEP-NETs) are characterized by high-density expression of somatostatin receptors (SSTRs). RYZ101 (225Ac-DOTATATE) is a highly potent alpha-emitting radiopharmaceutical therapy (RPT) being developed for SSTR+ solid tumors. ACTION-1 is a 2-part, global, randomized, controlled, open-label, Phase 1b/3 trial of RYZ101. Part 1 (Phase 1b), an uncontrolled dose de-escalation study, is complete with no dose-limiting toxicities.

Introduction: Targeted radiopharmaceutical therapy (RPT) has demonstrated potential in treatment of gastroenteropancreatic-neuroendocrine tumors (GEP-NETs). However, RPT exhibits variable outcomes and lacks tools to predict individual efficacy. To address the latter, a genomic profiling analysis was included in the randomised, controlled, open-label, phase III COMPOSE trial of Lu-177 edotreotide vs. best standard of care (CAPTEM, FOLFOX or everolimus) in patients with well differentiated, aggressive, G2/G3 somatostatin receptor-positive GEP-NETs.
