A case of functional pancreatic neuroendocrine tumor with hypercalcemia as the main manifestation

Introduction: NEN is a type of rare neoplasm originated from peptidenergic neurons and neuroendocrine cells. Even for functional neuroendocrine tumors, the hormones secreted can be temporally and spatially heterogeneous. Hypercalcemia being a rare clinical manifestation of F-pNEN.

Aim(s): -

Materials and methods: A 64 year-old man was admitted in March 2023 for recurrence of pancreatic NEN more than 8 years after surgery. In 2015 physical examination revealed pancreatic occupancy. March 2016 he underwent the lumpectomy, resection sites include pancreatic tail, spleen and retroperitoneal tumors. Pathological diagnosed as pancreatic NEN (G2), Ki-67 (localised up to 15%). In March 2019, due to enlargement of hepatic occupancy and oral Everolimus for 4 years. PET/CT (18F-FDG,68Ga-DOTANOC March 2023) suggests recurrent tumor in the stump pancreatic body, multiple metastases in the liver and abdominal cavity, glucose metabolism active and strong positive growth inhibitory receptor imaging. Pathology (liver puncture biopsy) diagnoses NEN with the grade of G2, Ki-67 was about 10%.First TAE treatment on March 2023 and Sandostatin LAR started after the surgery. Patient presented with pancreatitis induced by high calcium levels. Laboratory tests show that alcium 4.85mmol/L, parathyroid hormone5.17pg/ml.
