Early Response Assessment for Prediction of Overall Survival after Peptide Receptor Radionuclide Therapy

Introduction: PRRT response assessment is performed using RECIST, yet functional assessment using 68Ga-DOTATATE PET/CT and plasma CgA might give additional information about prediction of overall survival (OS).

Aim(s): To assess whether biomarkers like RECIST measurements, 68Ga-DOTATATE uptake and CgA determined at 3 and 9 months after PRRT are associated with OS.

Materials and methods: CT or MRI, 68Ga-DOTATATE PET/CT and CgA assessment were performed at baseline, 3 and 9mo after four PRRT cycles. RECIST target lesions were measured. 68Ga-DOTATATE uptake was evaluated using two methods: 1) follow-up of most intense baseline lesions and 2) assessment of the most intense lesions on each scan. OS was calculated as time from start of PRRT until death from any cause. Kaplan-Meier curves were plotted to compare median OS. Changes in imaging parameters and CgA measurements after 3 and 9mo after PRRT were evaluated using Cox survival analyses for association with OS.
