Efficacy of regional and systemic therapies for refractory hypoglycaemia in malignant insulinoma

Introduction: Insulinoma is a rare neuroendocrine tumour characterised by inappropriate autonomous insulin secretion.
Aim(s): This retrospective review evaluates the efficacy of regional and systemic therapies in managing refractory hypoglycaemia in metastatic or unresectable insulinoma.
Materials and methods: We reviewed patients treated for metastatic or unresectable insulinoma at Mayo Clinic (1992-2024). Hypoglycaemic severity was graded (0=asymptomatic, 1=diet-controlled, 2=requiring meds/procedures, 3=hospitalisation). Surgical interventions (e.g. tumour resection, debulking), regional therapies (e.g. ablation, embolisation), and systemic therapies were assessed for impact on hypoglycaemia.
