Genetic trajectory and clonal evolution of non-functional pancreatic neuroendocrine tumours with lymph node and liver metastasis

Introduction: The systemic spread of panNETs is dominated by lymphatic reflux and portal system, exhibiting diverse patterns of metastasis without systematical genomic investigation.

Aim(s): Here, we evaluated the genomic evolution of panNETs with multi-lesion metastases using whole-exome sequencing.

Materials and methods: Whole-exome sequencing was performed on multiple regions (n=90). Matched primary tumour, adjacent non-cancerous tissue, liver metastasis and lymph node metastasis from 17 patients with panNETs were sequenced. Phylogenetic reconstruction and evolutionary analyses were used to investigate the metastatic seeding pattern and clonal origin. Recurrent driver gene mutations were analysed across patients.
