Management of an Advanced Pancreatic Neuroendocrine Tumor

Introduction: Whereas an array of treatment modalities have evolved for management of pancreatic NETs, there has been a lack of information regarding a clear algorithm of optimal treatment or an appropriate sequence in which different treatment modalities can be applied to a given patient for optimal results.

Aim(s): We report a young woman with an invasive pancreatic NET that disseminated to regional lymph node and vertebra. We suggested and planned for her a treatment algorithm consisting of Whipple’s procedure, followed by concurrent use of radionucleide and Everolimus w/ XRT for the skeletal lesion.

Materials and methods: A 20-year-old female was referred to us post-Whipple procedure for a pancreatic NET. We undertook histological examination of the tumor tissue, I-131 MIBG scan, CT abdomen. Octreoscan, PET-CT. serum CGA & genetic studies are pending.
