Pitfalls in Glucagon-Like Peptide-1 Receptor Imaging

#2990

Introduction: Physiological pancreatico-duodenal DOTA-Exendin-4 uptake in the Brunner’s glands of the proximal duodenum occurs in Glucagon-like peptide-1 receptor (GLP-1R) imaging and is a known potential pitfall.

Aim(s): The aim of this study was to analyze pancreatico-duodenal uptake in GLP-1R SPECT/CT and PET/CT and identify other potential reading pitfalls in GLP-1R imaging.

Materials and methods: Data of a prospective study, including 52 consecutive patients, was used for this post-hoc evaluation of pitfalls in GLP-1R scans. All patients underwent one 68Ga-DOTA-exendin-4 PET/CT and two 111In-DOTA-exendin-4 SPECT/CT scans (4 and 72h p.i.) in a randomized crossover order. Reference-standard was surgery with histopathological confirmation of an insulinoma and normalization of blood glucose levels post-surgery.
