Post-PRRT imaging: Which scans to make and what to look for

Introduction: Post-PRRT imaging often focuses on dosimetry but seldom on its clinical utility.

Aim(s): To evaluate the clinical utility of different post-therapy 177Lu-DOTATATE planar and SPECT/CT acquisitions to simplify imaging protocols.

Materials and methods: 100 patients with a metastasized NET treated with PRRT were included. Post-therapy imaging was performed 24 hours after each cycle by whole body planar imaging and abdominal- and thoracic SPECT/CT. All images were evaluated for 1. presence of new lesions, 2. discordance between planar and SPECT, 3. location of new lesions, and 4. additional findings on non-contrast enhanced CT.
