Role of Somatostatin Analogues in the Management of Merkel Cell Carcinoma: A Case Report

Introduction: Merkel cell carcinoma (MCC) is a relatively rare neuroendocrine malignant tumor that is characterized by an aggressive course with regional nodal involvement, distant metastases and a high rate of recurrence.

Aim(s): To present a case of successful treatment of MCC.

Materials and methods: A 46-year-old female patient was referred for evaluation of a mass in the soft tissue of the left thigh. Tumor mass (8x12cm) and inguinal adenopathy were found. A wide excision and inguinofemoral lymphadenectomy of the left side were performed in December 2009. In immunohistochemical examination, the tumor cells showed diffuse positivity for Chromogranin A (CgA) and a characteristic dot-like staining for cytokeratin 20, Ki-67 – 50%. Diagnosis of MCC was established. Paraaortal adenopathy was diagnosed by CT scan in January 2010. Blood levels of CgA and serotonin were 33.2 U/L (N–2-18) and 425.2 ng/ml (N–30-200), respectively. The patient received six cycles of CT with cisplatin (70mg/m2 day 1) and etoposide (120mg/m2 days 1-3) since February to July 2010. Therapy with SS analogues (Sandostatin LAR 20mg every 28days) was started in March 2010.
