The Number of Positive Nodes Accurately Predicts Recurrence after Pancreaticoduodenectomy for Nonfunctioning Neuroendocrine Neoplasms

Introduction: The presence of lymph node metastases is one of the most powerful predictor of recurrence after surgery for pancreatic neuroendocrine neoplasms (PanNENs), but the most appropriate nodal staging for PanNENs is unclear.

Aim(s): Aims of the study were to evaluate the effect of the number of positive lymph nodes (PLN) on prognosis and to find a minimum number of lymph nodes to be resected/examined in patients after pancreaticoduodenectomy (PD) for PanNENs.

Materials and methods: Consecutive PDs performed with curative intent for nonfunctioning PanNENs at three different institutions (Johns Hopkins University Hospital, San Raffaele Hospital and Varese University Hospital) were retrospectively analyzed. PanNENs with nodal metastases (N+) were classified into N1 [1 to 3 positive lymph nodes (PLN)] and N2 (4 or more PLN). Univariate and multivariate analyses of disease free survival (DFS) were performed.
