Introduction: Metronomic schedule of temozolomide (mTEM) can overcome resistance induced by O6-methylguanine-DNA-methyltransferase (MGMT) and reduce toxicities being particularly suitable for unfit patients (pts). We present the first data from a non-planned interim analysis of this phase II Italian single arm multicenter study.

#3918 Efficiency of GemCap + mitotane in platinum and mitotane resistant adrenocortical carcinoma
Introduction: Gemcitabine, metronomic capecitabine and mitotane (M) combination (GemCap + M) is the most investigated and widely used regimen in 2nd and subsequent lines therapy of advanced adrenocortical cancer (ACC). In the largest retrospective study (N=145) only 28% of patients (pts) reached therapeutic concentration of mitotane (M), which translated into a significant improvement in the median progression free survival (PFS).

Introduction: Cabozantinib (Cbz), a tyrosine kinase inhibitor, and Temozolomide (TMZ), an alkylating agent, show relevant clinical implications in neuroendocrine neoplasms (NENS). In glioma patients, the combination of Cbz and TMZ was well tolerated even with the addition of radiotherapy. It is possible to hypothesize a synergistic combination of Cbz and TMZ. Cbz, through the inhibition of the c-MET pathway, could enhance the sensitivity of NENs to TMZ. In addition, the intermittent metronomic schedule of TMZ has a proven anti-angiogenic activity that could be additive and/or enhance the anti-angiogenic effect of Cbz.

Introduction: Second-line treatment for neuroendocrine carcinoma (NEC) have been investigated in prospective trials, with only few prospective trials ongoing. Therefore, there is no consensus on the second-line regimen to use in patients with progressive NEC. These patients have generally a poor performance status (PS) and the tolerance to combination therapy is low. Metronomic temozolomide showed, in retrospective trials, a good safety and efficacy profile.

Introduction: Results from phase II and non-randomized trials with metronomic 5-FU in combination with SSA in well-differentiated (WD) NET are limited, and considered investigational.
