Introduction: After progressing on somatostatin analogues (SSA), patients with somatostatin receptor positive gastroenteropancreatic (GEP) NETs often receive PRRT. It is unclear whether current practice of continuing SSA after commencing PRRT is beneficial, especially in non-functioning NETs.

Introduction: Peptide receptor radionuclide therapy (PRRT) with fixed activity dosing is an established treatment for advanced neuroendocrine tumors (NETs). Emerging data suggest that patients may benefit from a personalized treatment approach. Aggressive NET is believed to benefit from more intensive treatment, one example being FDG-PET positive tumors have shown benefit from combining PPRT with chemotherapy. Dosimetry is another means of achieving personalisation and can be used to increase the tumor radiation doses without surpassing the toxicity limits of organs at risk.

Introduction: Well-differentiated gastroenteropancreatic neuroendocrine tumors (GEP-NETs) are characterized by high-density expression of somatostatin receptors (SSTRs). RYZ101 (225Ac-DOTATATE) is a highly potent alpha-emitting radiopharmaceutical therapy (RPT) being developed for SSTR+ solid tumors. ACTION-1 is a 2-part, global, randomized, controlled, open-label, Phase 1b/3 trial of RYZ101. Part 1 (Phase 1b), an uncontrolled dose de-escalation study, is complete with no dose-limiting toxicities.

Introduction: Scottish National PRRT Service delivers 177Lu-Dotatate nationally at the Beatson West of Scotland Cancer Centre (BWoSCC). The NETTER-1 trial (Strosberg et al 2017) significantly changed the treatment and management of patients with gastroenteropancreatic NET (GEPNET). 177Lu-Dotatate demonstrated superiority in progression free survival (PFS) in patients with G1 and 2 midgut neuroendocrine tumors (NET). 177Lu-Dotatate 7.4 GBq 8 weekly gave 65% estimated progression free survival (PFS)at 20 months versus 10.8 % with 60mg octreotide intramuscularly 4 weekly (Strosberg et al 2017). Prior to this study everolimus was the most common 2nd line treatment for SBNET with a PFS of 14 months (Puscuddo et al 2017).
