Combination of Surgery and Ethanol Ablation in a MEN-2A Patient

Introduction: MEN-2A is an autosomal dominant syndrome characterized by pheochromocytoma, medullary thyroid carcinoma and hyperparathyroidism.
Aim(s): To present a case of MEN-2A treated by combination of surgery and ethanol ablation.
Materials and methods: A 49-year-old male patient with paroxysmal hypertension for 5 years, who got bilateral adrenal pheochromocytoma resection in 1989, came back our hospital in 2006. He suffered from headache, palpitation, and sweating, BP up to 230/140 mmHg. Due to inefficient treatment by drugs such as SNP, NIF, and ACEI, a recurrent PHEO had been considered. PE:a 2*2cm mass in the left lobe of thyroid. Lab tests: Catecholamine elevated ( NE 87.70 (0.51-3.26); E 1.89 (0.05-1.39), iPTH 81.1 pg/ml (10-69), PCT>1600 pg/ml (<100), T3 and T4 normal level. CT scan showed low density masses in bilateral adrenal,nodular opacities in the left lobe thyroid. Thyroid needle biopsy was performed and the report was medullary thyroid carcinoma (left).
