Follow-up is not justified in patients with typical lung carcinoids treated with curative intent

Introduction: Typical carcinoids (TC) are well-differentiated neuroendocrine neoplasms of the lung with generally favourable prognosis. Recurrence is rare in TC patients treated with curative intent, yet current ENETS guidelines recommend extensive follow-up, including regular and long-term imaging.
Aim(s): This study investigates the risk of recurrence in TC patients treated with curative intent and assesses the justification of the current follow-up programme.
Materials and methods: We identified all patients with TC from 2009 to 2020 at two Danish ENETS Centers of Excellence: Copenhagen University Hospital - Rigshospitalet, Copenhagen, and Aarhus University Hospital, Aarhus. Patient data were extracted from electronic patient records. We included all patients treated with curative intent and without lymph node involvement, i.e. M0, N0 disease and R0 resection. Follow-up continued from surgery until recurrence, death, or administrative end of follow-up. We used the Aalen-Johansen method to estimate the cumulative incidence of recurrence, treating death as a competing risk.
