Genetic Analysis Identifies Subgroups of Small Well-Differentiated Pancreatic Neuroendocrine Tumors with Different Risk for Liver Metastases

Introduction: Small size well-differentiated pancreatic neuroendocrine tumors (PanNETs) behave non-aggressively. However, a limited subset progresses with liver metastases.

Aim(s): This study aimed to investigate molecular alterations associated with metastasis.

Materials and methods: 87 primary PanNETs <3cm, including 32 metastasizing to liver and 55 non-metastatic after a 5-year follow-up were immunostained for DAXX and ATRX and analyzed for alternative lengthening of telomeres (ALT) by fluorescent in situ hybridization. A subset of 48 cases, 24 metastasizing and 24 non-metastasizing were subjected to mutational analysis of 18 genes by targeted next-generation sequencing and to whole-genome copy number variation (CNV) analysis.
