Metastatic non-functional pancreatic neuroendocrine tumour with transformation to insulinoma shown on 68Ga-Exendin-4 PET/CT

Introduction: 68Ga-exendin-4 PET/CT imaging has been utilised for benign insulinomas. However, some reports demonstrated the absence of GLP-1R expression of malignant insulinomas.
Aim(s): We present a case of a non-functional metastatic pancreatic neuroendocrine tumour that transformed into an insulinoma primely demonstrated by 68Ga-Exendin-4 PET/CT.
Materials and methods: A 52-year-old man was pathologically diagnosed with a pancreatic neuroendocrine tumour (pNET) and synchronous liver metastasis by debulking surgery 2 years ago. The patient showed recurrent severe hypoglycaemia recently after receiving multiple lines of treatment. 68Ga-DOTANOC and 68Ga-exendin-4 PET/CT were used to assess the metastatic lesions of the whole body due to the tumour progression and uncontrolled hypoglycaemia symptoms.
