Theranostic Implication of Molecular Imaging Phenotype of Well-Differentiated Lung Carcinoid by 68Ga-DOTATATE PET/CT and 18F-FDG PET/CT

Introduction: Well-differentiated pulmonary neuroendocrine tumour, classified as typical carcinoid (TC) and atypical carcinoid (AC), is a heterogenous disease with variable clinical behaviour.

Aim(s): To analyse the molecular imaging (MI) phenotype of TC and AC by 68Ga-DOTATATE (GaTATE) and 18F-FDG (FDG) PET/CT with the emphysis on its potential theranostic implication for peptide receptor radionuclide therapy (PRRT).

Materials and methods: Retrospective review of patients with biopsy-proven TC or AC with both GaTATE and FDG at presentation analyzed qualitatively and semi-quantitatively. Positive lesions were defined by the theranostic concept as uptake > liver on both tracers. Per-patient dual tracer analysis was classified as: score 1) both -ve in all lesions, 2) GaTATE +ve/FDG -ve, 3) both +ve, or 4) GaTATE -ve/FDG +ve in some or all lesions. Score 1 and 4 was deemed unsuitable for PRRT.
