#4228 Evaluation of the impact of indoleamine 2,3-deoxygenase (IDO) enzyme activity in neuroendocrine tumors (NETs)

Introduction: Indoleamine 2,3-deoxygenase (IDO) enzyme, by inducing L-tryptophan (Trp) depletion and L-kynurenine (kyn) accumulation, contributes to the formation of an immunosuppressive microenvironment. Some studies highlighted that patient with cancer in treatment with immune checkpoint inhibitors (ICI) and with low Kyn/Trp values showed a better PFS and OS, but there are no data for NETs.

#3483 A phase II single arm interventional trial evaluating the activity and safety of combination between CABOzantinib and TEMozolomide (TMZ) in lung and GEP-NENs progressive after SSAs, everolimus, sunitinib or PRRT-CABOTEM

Introduction: Cabozantinib (Cbz), a tyrosine kinase inhibitor, and Temozolomide (TMZ), an alkylating agent, show relevant clinical implications in neuroendocrine neoplasms (NENS). In glioma patients, the combination of Cbz and TMZ was well tolerated even with the addition of radiotherapy. It is possible to hypothesize a synergistic combination of Cbz and TMZ. Cbz, through the inhibition of the c-MET pathway, could enhance the sensitivity of NENs to TMZ. In addition, the intermittent metronomic schedule of TMZ has a proven anti-angiogenic activity that could be additive and/or enhance the anti-angiogenic effect of Cbz.

#701 High Expression of Mammalian Target of Rapamycin (mTOR) in NF1-Related Periampullary Neuroendocrine Tumors

Introduction: Neurofibromatosis type 1 (NF1) is a rare inherited disorder associated with cutaneous lesions and neoplasms in the nervous and gastroenteropancreatic systems, including periampullary NETs. NF1-coded protein, neurofibromin, is a tumor suppressor that negatively regulates the mammalian target of rapamycin (mTOR). Preclinical in vitro and in vivo studies demonstrated a constitutive activation of mTOR pathway associated with significant antineoplastic effects of mTOR inhibitors in NF1-related glial tumor models.
