#4544 Spatial and temporal intratumour heterogeneity of pancreatic neuroendocrine tumours (PanNET)
Introduction: Data derived from the study of intertumour heterogeneity have led to propose that PanNET evolution may start either from beta cells with good prognosis, or from alpha cells with sequential alterations of MDA (MENIN-DAXX/ATRX) leading to more aggressive lesions.

Introduction: HG-GEP-NEN are classified into neuroendocrine tumours grade 3 (NET G3) and neuroendocrine carcinomas (NEC). This distinction is crucial as it affects patient prognosis and treatments but can be challenging.

Introduction: NENs are rare tumours which can benefit from specialised pathological assessment. In France, the ENDOCAN/TENpath network, composed of 33 expert pathologists in 20 centres, provides consultations (2nd readings) at the request of non-specialist pathologists. Challenging cases are reviewed in monthly virtual meetings grouping all expert pathologists (third readings).

Introduction: Patients (pts) with von Hippel-Lindau (VHL) disease need novel targeted therapies. Hypoxia-inducible factor 2α (HIF-2α) is a key oncogenic driver in VHL disease. Belzutifan (MK-6482), a HIF-2α inhibitor, has shown antitumor activity in pts with advanced renal cell carcinoma (RCC) and pancreatic neuroendocrine tumors (pNETs); more data are needed in VHL disease–associated localized tumors.

Introduction: Epidrugs like DNA methyltransferase inhibitors (DNMTi) can increase somatostatin receptor type 2 (SST2) expression in neuroendocrine tumor (NET) cells in vitro and in vivo. This effect could be used for NET patients with low SST2 expression who are currently ineligible for somatostatin analogue (SSA) treatment. However, the DNMTi known to stimulate SST2 have either a high toxicity profile or are not yet approved.
