Approval of a programme to perform 50 liver transplants for neuroendocrine tumor liver metastases in UK and Ireland affords significant opportunities for clinical study and research

Introduction: Liver transplantation for unresectable neuroendocrine tumor liver metastases [NET LM] remains a confusing and controversial area in managing NETs. Patients deemed most suitable for transplantation have good prognosis with standard therapy and best outcomes with transplantation are only achieved at highly experienced Centre(s).
Aim(s): To develop infrastructure for this highly demanding aspect of NET patient management, to understand the pitfalls, develop scientific collaborations and clinical trials, ultimately to deliver a randomised clinical trial of transplantation v standard care. The programme will identify patients that can benefit from liver transplantation and develop a safe and effective service for them.
Materials and methods: Patients are to be selected based on the Milan criteria for liver transplantation for NET LM. Initial 10 transplants will be strictly within the published Milan criteria with subsequent 40 transplants having the ability to exceed the criteria without compromising transplant benefit – as judged by the National Board that will oversee the programme. Istituto dei Tumori Milano will be a collaborative Centre and other Centres will have the opportunity to also participate, assessing patients, transplanting and collecting data under the auspices of the UK and Ireland National Board.
