Introduction: Pancreatic neuroendocrine neoplasms (PanNENs) show heterogeneous behavior, mainly related to their grade (G), nevertheless small well-differentiated G1 and G2 lesions are usually approached the same conservative way.

Aim(s): To explore the clinical features, therapeutic options, and prognosis of G2 PanNENs, and to compare them with G1.

Materials and methods: This is an observational, retrospective, single-center study examining G2 PanNENs patients from January 2005 until December 2022. Demographic, clinical, biochemical, histopathological, imaging, therapeutic, and prognostic data were collected and compared to G1 PanNENs patients.
