Metastatic middle ear neuroendocrine tumor treated with Somatostatin analogues and PRRT – A case report

Introduction: Neuroendocrine adenomas of the middle ear (NEAME) are rare tumors. They have mostly indolent biological behaviour, however, reports of an aggressive course and metastasis have been described.
Aim(s): To describe a case of NEAME treated with somatostatin analogues (SSA) and peptide receptor radionuclide therapy (PRRT).
Materials and methods: 72-year-old man with past history of a middle ear tumor submitted to surgery. After 6 years, recurrence of the disease was detected and he underwent right subtotal petrosectomy, total parotidectomy and radical neck and parapharyngeal lymph node dissection. Histology revealed a middle ear neuroendocrine tumor with metastatic cervical lymph nodes (Ki-67 SI:6%). Macroscopic positive margins were observed. Ga-68-DOTA-NOC PET/CT scan performed later exhibited high uptake in the temporal bone, latero-cervical region, retropharyngeal soft tissues and C1 vertebra suggesting metastasis.
