#4245 Carcinoid heart in neuroendcrine tumor – Experience of medical departement oncology Oran, Algeria

Introduction: The carcinoid heart is present in approximately 20% of cases at the diagnosis of carcinoid dyndrome in neuroendocrine tumors, and will occur in approximately 50% of patients during follow-up. The consequence is the appearance of stenosing and/or leaking valvular diseases, responsible, eventually, right or left ventricular dysfunction and severe heart failure. Only surgical valve replacement treatment seems to be able to improve the prognosis, but at the cost of significant perioperative morbidity and mortality.

Introduction: Chronic heart failure leads to cachexia in 5-15% of patients. Malnutrition affects 58% of patients with carcinoid syndrome.

#3517 The role of the NET clinical nurse specialist in managing carcinoid heart disease patients: Optimising patient care

Introduction: The diagnosis of carcinoid heart disease (CHD) is often made late and requires a highly experienced team to manage multiple issues, such as heart failure, carcinoid symptoms and poor nutrition. Prior to December 2018 NET (Neuroendocrine Tumor) and cardiac teams were working in isolation, this resulted in missed opportunities for intervention and comprehensive review. It was evident we needed to build closer collaborations with our cardiac colleagues, to ensure timely assessment, optimisation of symptoms and improve patients’ overall experience. The Clinical Nurse Specialists (CNSs) were fundamental in establishing a twice monthly CHD/NET multidisciplinary team meeting and monthly joint NET/CHD clinic. Patients were kept informed at each stage by the CNS, either during telephone consultation, ward visit or at clinic appointments.

#3313 Carcinoid heart surgery: Review of outcomes from a single large centre

Introduction: Up to half of patients with carcinoid syndrome develop carcinoid heart disease and valve degeneration can lead to right heart failure. Surgical replacement of affected valves is an effective therapy. Peptide receptor radio-targeted therapy (PRRT) has demonstrated better progression-free survival compared to high dose sandostatin LAR.

#2944 Soluble ST2 (sST2) Levels in the Management of Carcinoid Heart Disease in Patients with Neuroendocrine Cancer

Introduction: The identification of a sensitive biomarker to detect the presence and severity of carcinoid heart disease could allow more selective use of clinical and echocardiographic screening. ST2 is a protein belonging to the family of interleukin 1 receptors, present both in the isoform of transmembrane (ST2L) and in the soluble one (sST2). sST2 blocks the anti-inflammatory, anti-hypertrophic, and anti-fibrotic effects of cytokine IL-33 on the myocardium and recent studies have shown that sST2 levels increase in the blood of patients diagnosed with heart failure.
