#4021 What is the carcinoid syndrome? A critical appraisal of its proposed mediators
Introduction: Carcinoid syndrome (CS) is a debilitating disease that affects approximately 20% of patients with neuroendocrine neoplasms (NEN). Due to the increasing incidence and improved overall survival of patients with NEN over recent decades, patients are increasingly suffering from chronic and refractory CS symptoms. At present, symptom control is hampered by an incomplete understanding of the pathophysiology of this syndrome.

Introduction: Carcinoid Heart disease is a common disease with neuroendocrine malignancies that invade the liver due to the paraneoplastic effects of vasoactive substances such as 5-hydroxytryptamine, histamine and tachykinins released by malignant cells rather than any direct metastatic involvement of the heart.

#423 Case Report : A Rare Cause of Retroperitoneal Fibrosis Responding to Steroids
Introduction: Retroperitoneal fibrosis is a rare inflammatory disease. Neuroendocrine tumors are a heterogenous group that arise from enterochromaffin cells, most commonly in the gastrointestinal tract. Their capacity to secrete hormones, five HT, tachykinins etc, are responsible for symptoms known as carcinoid syndrome. However NETS, particularly arising from the midgut, can cause fibrosis. This can result in bowel obstruction, but also cause fibrosis in the retroperitoneum, lungs and cardiac valvular fibrosis.

Introduction: Carcinoid heart disease (CHD) affects up to 15-70% of patients with carcinoid disease. Recent ENETS guidelines recommend regular echocardiographic assessment to screen for CHD. Chromogranin A (CgA) is a peptide widely used to monitor carcinoid disease progression. Neurokinin A is a tachykinin which may be a more sensitive measure of carcinoid disease progression and prognosis. N-terminal pro Brain Naturetic Peptide is used as a screening tool for heart failure and recent results suggest utility in the diagnosis of CHD.
