Clinical application of digital pathology: Proposal of a new G0 category useful in small intestinal NET (SI-NET)

Introduction: SI-NET display low proliferation rate despite a high frequency of metastases. Ki67 index is frequently reported as “<3%” in pathological reports of G1 SI-NET, without precise quantification.

Aim(s): We explored whether precise Ki67 assessment with digital pathology could help better predicting the prognosis of patients with G1 SI-NET.

Materials and methods: We included 64 consecutive patients operated on in our institution from 2001 to 2018 for a G1 SI-NET. Ki67 index was evaluated on all primary tumors (PT) ± lymph nodes (LN) ± metastases (LM), by image analysis (Qupath) in small (0.4 mm2; Ki-S) or large (2 mm2; Ki-L) hotspots. Our primary endpoint was recurrence-free survival (RFS).
