Performance of a prognostic OTP, CD44, Ki-67 biomarker panel on paired biopsies and resections of lung carcinoids

Introduction: Previously OTP, CD44 and ki-67 have been identified as prognostic biomarkers for lung carcinoid (LC).

Aim(s): To examine prognostic biomarker performance in LC biopsies (Bx) compared to paired resection (Rx) specimens.

Materials and methods: Patients with LC (stage I-III, 2003-2012) who underwent curative resection were selected from Dutch pathology archives. 119 paired Bx-Rx cases were collected, of which n=71 cases were eligible for biomarker evaluation. WHO 2021 classification revision was applied. Atypical carcinoid (AC) histology is defined as the presence of 2-10 mitoses per 2mm² and necrosis. H-score was used for OTP and CD44 expression, and ki-67 proliferation index was scored by eyeball estimation with hot-spot scoring. Patients were classified as low relapse risk (OTP≥50 and CD44≥30 and Ki67<5) or high relapse risk (all others).
