Introduction: LOLA is an Italian, multicentre, open-label, double cohort, non-randomised, 3-stage, phase 2 trial aiming to assess the safety and activity of the combination of cabozantinib (CABO) + lanreotide (LAN) in patients (pts) with advanced or metastatic GEP and unknown primary NETs with Ki-67 > 10% or thoracic NET. In NETs sunitinib is the only TKI approved in advanced pancreatic NETs. While CABO has been reported superior to sunitinib in renal cancer its role in NETs is still investigational.

Introduction: Everolimus and Sunitinib are often used consecutively for advanced, progressive, well-differentiated pancreatic neuroendocrine tumors (pNET), but to date no data on the optimal sequence is available.

Introduction: To be suitable for [177Lu]Lu-DOTA-TATE, SSTR2 must be present on tumor surface as per positive uptake on [68Ga]Ga-DOTA-TATE-PET/CT. SSTR2 promoter methylation reduces SSTR2 expression; this can be reversed using a demethylating agent.

#3870 Small bowel neuroendocrine tumor with carcinoid heart disease
Introduction: Patients with neuroendocrine tumors are often misdiagnosed, at least for the first stage of the disease. In fact, carcinoid syndrome is the clinical expression of the vasoactive substances produce. Carcinoid heart disease is a hazardous and irreversible complication especially in late identified cases.

Introduction: Previous global results of ORR and PFS1 have been presented. The best treatment (Tx) sequence between STZ-5FU and everolimus (EVE) in patients (pts) with advanced and progressive panNETs is not yet well established.
