Medullary thyroid carcinoma and somatostatin analogues, a possible therapeutic choice in asymptomatic and slow radiological progression metastatic recurrent disease after surgery – Case report of a 22-years long term survival patient
Introduction:
Medullary thyroid carcinoma (MTC) is a well-differentiated neuroendocrine tumor of the thyroid and accounts for 1%-2% of thyroid cancers in the United States. MTCs present with locoregional metastasis in 50% of patients and distant metastasis in 10%-15% at diagnosis. Treatment options for recurrent or progressive disease according to RECIST 1.1 criteria, are limited. Tyrosine kinase-inhibitors (TKI) are preferred as first-line systemic therapy. Although somatostatin analogues (SSA) may play a role in MTC, limited evidence is available.

The role of 18F DOPA-PET in a case of malignant pheochromcytoma
Introduction:
Pheocromocytoma is a rare tumor arising from chromaffin cells of adrenal medullary or extra adrenal paraganglionic tissue. Histological criteria cannot differentiate benign from malignant pheocromocytomas. The diagnosis of a malignant pheocromocytoma requires local invasion, recurrence and documented metastatic disease. The radionuclide scanning (123I - 131I-MIBG) is a fundamental diagnostic tool used to confirm the biochemical and radiological diagnosis of pheochromocytoma. In fact, MIBG scanning may confirm that the visualised lesion in an adrenal gland is indeed a pheochromocytoma and detect extra-adrenal paraganglionic tissue. However, MIBG scans are negative in around 15% of benign pheochromocytomas and in up to 50% of malignant ones. Other radionuclide techniques (18FDG-PET, 18F-DOPA-PET, 18F-FDA-PET) have been successfully used in investigation of pheochromocytomas. 18F-DOPA-PET and 18F-FDA-PET have been reported to be highly sensitive and specific for benign pheochromocytomas, while 18FDG-PET can be useful for malignant lesions with higher metabolic activity.

The importance of endoscopic ultrasound in detecting recurrent gastrinoma in a case of MEN 1
Introduction:
Approximately 25-30% of patients have gastrinomas as part of the inherited syndrome Multiple Endocrine Neoplasia 1 (MEN 1). Gastrinomas occur in the pancreas, duodenum or peripancreatic lymph nodes. Diagnosis is made by clinical history, gastroscopy, and measurement of serum gastrin, gastric juice pH, CT scan, endoscopic ultrasound (EUS) and somatostatin receptor scintigraphy (SRS). Localization of gastrinomas in patients with MEN 1 is challenging due to their small size, frequent duodenal location, and multiplicity.
