#4418 Endoscopic ultrasound-guided radiofrequency ablation is a safe and efficient treatment of symptomatic insulinomas – A single centre experience of 14 patients

Introduction: The vast majority of insulinomas are benign and less than 20 mm. Surgery is considered as standard treatment, but complications are significant. Endoscopic ultrasound-guided radiofrequency ablation (EUS-RFA) represents a novel and minimally invasive approach.

#4387 Exenatide-test for diagnosing endogenous hyperinsulinemic hypoglycaemia: A randomised placebo-controlled cross-over proof-of-principle study – FAST study

Introduction: The gold standard to diagnose endogenous hyperinsulinemic hypoglycaemia (EHH) is the fasting test which is cumbersome and incurs high costs. In the setting of radio-labelled 68Ga-exendin-4 PET/CT in patients with EHH (to localise insulinoma), we unexpectedly observed clinically relevant hypoglycaemia following administration of the tracer, which does usually not occur after GLP-1 agonist therapy in type 2 diabetic patients and in healthy subjects.

#3877 Short fasting test as a reliable and effective tool to diagnose insulinoma

Introduction: Insulinomas are pancreatic NETs presenting with hypoglycaemia. Current guidelines for its diagnosis require a prolonged fasting test (72 hours), which implies hospitalization and is difficult to perform. It has been reported that up to 85% of insulinomas are diagnosed within a 24h fasting period.

#3324 Elastographic localization of insulinoma

Introduction: A 15-year-old healthy female, initially admitted with less serious signs of hypoglycemia. Laboratory studies showed a glucose concentration of 1.2 mmol/l and insulin 9.6 μU/ml. Complete blood count, basal metabolic profile, activity of C - reactive protein and urine drug screen, were unremarkable. Fasting test confirmed biochemical signs of insulinoma.

#1074 Usefulness of a Combination of a 48-Hour Fasting and a Glucagon Tolerance Test

Introduction: The 72-hour fasting test is the gold standard for the diagnosis of insulinoma. However, the test can become a burden for both patients and medical staff and, shortening the duration of fast has been attempted. There are several reports on the high sensitivity of fasting test but only a few reports on its specificity. An insulinoma can be localized using invasive tests such as endoscopic ultrasonography and selective arterial secretagogue injection test. Therefore, insulinoma detection with high specificity may be needed to avoid unnecessary examinations.
