Introduction: The incidence of gastroenteropancreatic neuroendocrine tumors (GEP-NETs) is increasing globally, and the patients are often diagnosed with synchronous liver metastases. Despite recommendations for primary tumor resection (PTR) with or without liver resection (LR) in the patients, there are conflicting data for their impact on overall survival (OS).

Introduction: Liver metastases (NELM) occur in nearly half of NET pts, 80%are unresectable, and affect their prognosis. Surgery remains the most effective option; curative liver resection (LR) and liver transplantation (LT) offer the best long-term outcomes with difficult to show outcome differences due to the long course of the disease. The risk of tumor recurrence is considerable after both approaches.

Introduction: Even though the relative indolent character of neuroendocrine neoplasms, untreated NELM have a detrimental impact on survival outcomes. Therefore, complete resection is recommended if technically possible.
