Introduction: Temozolomide (TMZ), an alkylating agent, is an effective treatment used in multiple tumor types including advanced pancreatic neuroendocrine tumors (PanNET). In glioblastomas, TMZ induces a hypermutator and hyperprogressor phenotype in a subset of patients through MMR deficiency. In advanced colon cancer, TMZ priming increases tumor mutational burden (TMB), favouring immunotherapy efficacy.

Introduction: PRRT has a major place in the therapeutic strategy of metastatic NETs. Its efficacy is generally evaluated after four cycles, with no early predictor identified to date. During each PRRT cycle, 177Lu-related gamma emissions can be measured by post-PRRT scintigraphy.

Introduction: Rb and p53 status may help distinguishing well from poorly differentiated neuroendocrine neoplasms (NEN) and may predict response to chemotherapy. Studies correlating Rb/p53 genetic and immunohistochemistry were performed on high-quality frozen samples with high throughput techniques efficient for copy number determination. This situation differs from routine practice in which small deletions may be hard to detect on formalin-fixed paraffin-embedded (FFPE) samples with a gene panel approach.

Introduction: Temozolomide (TMZ) and dacarbazine (DTIC) are alkylating agents (ALK) with antitumor efficacy in metastatic PanNET. Rechallenging ALK following prior efficacy is commonly performed but has not been evaluated.
