Introduction: Merkel cell carcinomas (MCC) are a rare, aggressive skin neuroendocrine carcinomas, with peak incidence in the elderly. MCC often presents as a firm, red/purple painless nodule with a short history of increasing size. Surgery is considered the 1st line treatment but elderly patients, in particular, have comorbidities which may preclude radical surgery requiring reconstruction. In advanced MCC, immunotherapy (IO) is 1st line systemic treatment. Chemotherapy is used 2nd line or if contraindications for IO but can be challenging in an elderly population. There may be an emerging role for PRRT due to the presence of somatostatin receptors on MCC. Radiotherapy (RT) can be used for non-surgical candidates, or those with unresectable or metastatic disease. Adjuvant post-operative irradiation (PORT) may provide additional benefit in risk reduction and improves local control.

#4628 Triple negative breast neuroendocrine carcinoma in older patient: A rare entity
Introduction: Neuroendocrine breast carcinoma is a rare histological form of breast cancer, that represent 0.1% of all breast cancers and is recognised by the World Health Organization as a distinct entity. Characterised by a non-specific clinical and radiological presentation. Therapeutic management is not codified and is usually superimposed on that of classical breast cancer.

Introduction: The management of small, asymptomatic non-functional pancreatic neuroendocrine neoplasms (NF-pNENs) is contentious, with two main approaches: surgical resection and "watch-and-wait." Current guidelines recommend monitoring NF-pNENs under 2 cm based on retrospective data, although recent U.S. studies suggest improved survival with surgery for 1-2 cm NF-pNENs. Some suggest the "watch-and-wait" approach for older patients, though its efficacy is unclear.

Introduction: Thymic neuroendocrine neoplasms (TNENs)are rare and heterogeneous diseases. With the aging of society, there has been an increase in the proportion of elderly patients with TNENs,however, research on these patients is limited.

#4247 Diffuse idiopathic pulmonary neuroendocrine cell hyperplasia - A case report
Introduction: Idiopathic Diffuse Neuroendocrine Diffuse Neuroendocrine Hyperplasia is a rare, but increasingly recognized entity that primarily affects middle-aged and elderly women, Of available therapies, somatostatin analogs and mTOR are the most promising
