Introduction: Pancreatic neuroendocrine tumours (pNETs) are a heterogenous group of pancreatic malignancies with a unique biology and pathophysiology. Tumour grade and extension are commonly used for prognostic determination. Current pathologic grading system needs regular updates to refine prognostic classification. Genomic screening may provide more objective classes and reflect tumour biology.

Introduction: High-grade pancreatic neuroendocrine carcinoma (panNEC) pose challenges in terms of classification and treatment. While molecular genetics have made strides in deciphering significant inter-tumor heterogeneity, the extent of intra-tumoral heterogeneity and lineage plasticity remains unexplored.

Introduction: Mesenteric fibrosis (MF) affects up to 50% of small intestine neuroendocrine tumor (SI-NET) patients, causing significant morbidity and mortality. MF pathophysiology is poorly understood, limiting treatment development and biomarker identification.

#4021 What is the carcinoid syndrome? A critical appraisal of its proposed mediators
Introduction: Carcinoid syndrome (CS) is a debilitating disease that affects approximately 20% of patients with neuroendocrine neoplasms (NEN). Due to the increasing incidence and improved overall survival of patients with NEN over recent decades, patients are increasingly suffering from chronic and refractory CS symptoms. At present, symptom control is hampered by an incomplete understanding of the pathophysiology of this syndrome.

Introduction: Dysregulation of alternative splicing has emerged as a new, overarching hallmark of cancer, which may arise from mutations and/or altered expression of specific components of the splicing machinery. To date, no studies have been reported comparing the spliceosomic landscape, i.e. the status of the splicing machinery and alternative splicing patterns, among the different histological types of pulmonary neuroendocrine neoplasms (LungNENs), namely typical and atypical pulmonary carcinoids, Large Cell Neuroendocrine Carcinoma (LCNEC) and Small Cell Lung Cancer (SCLC).
