#4609 External validation of Rachel score: A prognostic tool for lung neuroendocrine tumours
Introduction: Lung NETs are a heterogeneous group of tumours with variegated clinical presentation and course. The histological subtype (TC vs AC) and TNM stage are established prognostic factors. In a previous work by our group, we developed a prognostic model named Rachel score, with a satisfactory ability to predict OS and PFS in this population.

Introduction: Post-transcriptional modifications play crucial roles in regulating gene expression and cellular function. While recent findings indicate that alternative splicing dysregulation plays a key role in tumorigenic properties of neuroendocrine tumours (NETs), the role of other RNA processing mechanisms, such as N6-methyladenosine (m6A) — being the most common RNA methylation — remains largely unexplored in NETs.

Introduction: The NETTER-2 trial showed a significant progression-free survival (PFS) benefit with 1L 177Lu-DOTATATE vs. high dose octreotide for patients with advanced G2/G3 GEP-NETs. No published evidence exists comparing 177Lu-DOTATATE with other recommended 1L treatments for patients with G3 GEP-NETs, such as capecitabine + temozolomide (CAPTEM), hence indirect comparisons need to be conducted.

Introduction: TNM 8 is the staging system for GEP-NEN, guiding prognosis and treatment. However, it does not include important prognostic factors such as age, sex, race, and morphology.

Introduction: Surgery is the curative therapy for lung neuroendocrine tumours (NET). Follow-up protocols after surgery are not evidence based and factors affecting survival are not clear.
