#4502 Molecular analysis of non-functioning PanNETs of the alpha lineage reveals new subtypes and mechanisms of progression

Introduction: Most pancreatic neuroendocrine tumours (PanNETs) are non-functioning. Previously, we demonstrated that small, MEN1-only mutated α-like PanNETs can be distinguished from larger ADM (mutated in ATRX, DAXX and MEN1) PanNETs based on epigenetic profiles. ADM PanNETs have shorter disease-free survival and a higher relapse risk. However, their therapeutic responses vary, underscoring group heterogeneity. ADM PanNETs remain insufficiently characterised, with potential subtype-specific progression drivers with implications for treatment choice and clinical outcome.

#3910 Integrative molecular analysis of lung neuroendocrine neoplasms with different Ki-67 indices identifies a molecular transition group between low- and high-grade neoplasms

Introduction: In the 2021 World Health Organization (WHO) classification of lung cancers, lung neuroendocrine neoplasms (LNENs) are classified as low-grade, intermediate-grade and high-grade based on mitotic count but not Ki-67 index, as for GEP-NENs.

#3872 Mixed neuroendocrine-non neuroendocrine neoplasms (MiNEN): Characterization of the genomic and pathological profile

Introduction: MiNENs is a novel defined rare entity, made up of two morphologically distinct neoplastic components; one neuroendocrine type (NEN), and the other one non-neuroendocrine type (nNEN). As available data are limited, the epidemiology and pathogenesis of MiNENs remain mostly unknown.

#3804 Wide molecular analysis of PanNET revealed that only MGMT promoter methylation seemed to correlate with Temozolomide response

Introduction: Temozolomide (TMZ) is an alkylating agent and is widely used in the treatment of PanNET. So far, only the MGMT status, defined by its immunohistochemical (IHC) expression or its promoter methylation, has been proposed as predictor of response to TMZ, but its value remains controversial.

#3513 Comprehensive molecular analysis of prognostic groups in Neuroendocrine Neoplasms (NENs) identifies key genes and potential regulatory mechanisms

Introduction: MiRNAs are key regulators of multiple molecular processes, including tumorigenesis and cancer progression. However, very few studies have assessed their role in NENs. In a previous study, we identified a miRNA signature that defined 3 prognostic groups in 86 patients with lung and gastroenteropancreatic (GEP) NENs. Here, we explore up- and downstream mechanisms underlying this miRNA signature.
